🧑‍⚕️ Qualified Social Worker – Adult Services
📍 Location: Merseyside | 💷 £32 per hour
🕒 Agency Contract | Full-Time via Pertemps Social Care

We're seeking experienced Qualified Social Workers to join a dedicated Adult Services Team in Merseyside, working across both community settings and hospital discharge services. 💼 The Role
As part of the Adult Services Team, you will:

Conduct holistic assessments of adults’ care and support needs, ensuring safety, dignity, and independence.
Work in accordance with social care legislation and local policies.
Manage a diverse caseload while delivering high-quality interventions.
Coordinate hospital discharges, working collaboratively with health partners.
Develop and implement individual care plans, promoting empowerment and respecting diversity.
Build and maintain meaningful, professional relationships with service users, carers, and other agencies.
Provide guidance to unqualified staff and students, sharing your experience and supporting development.

✅ What We're Looking For
We're seeking a committed professional who is:

A Qualified Social Worker registered with Social Work England.
Holding a recognised qualification (BA/MA/BSc/MSc in Social Work, DipSW, CQSW, or equivalent).
Experienced in working with adults in social care settings, particularly in hospital discharge or community roles.
Skilled in risk assessment, care planning, and multi-agency working.
Up to date with relevant legislation, guidance, and best practice.
Proactive, flexible, and focused on sustainable outcomes for service users.
Possessing excellent communication, mediation, and negotiation skills.
A strong advocate for adults’ rights, dignity, and independence.

Minimum requirement: 4+ years post-qualification experience.
