Adult Senior Practitioner, Hospital Discharge Team, Brent  
Candidates must be used to working in a fast paced team - need to be able to complete assessments and paperwork for a case in 2 working days. Must have comprehensive understanding of the Care Act & the Mental Capacity Act as well as general healthcare. 
At the start of the assignment, candidate will be expected to be in the office 5 days a week before moving onto a hybrid working set-up - this is for training and induction purposes.  Thereafter will be 3 days in the office and 2 wfh.  Please note, Brent work with hospitals in and out of Borough; will also be required to complete home visits as team deals with both in hospital and post discharge clients. 
£36 per hour
