Multiple roles available 
To support a proactive and responsive review of residents in care home where 1:1 support is requested and / or in place to ensure this is appropriate, least restrictive and work with providers to reduce this. 
To support a proactive and responsive review of individuals in receipt of Direct Payments care and support needs to ensure the package of care is appropriate. 
  
All roles involved visiting individuals in their home environment to review and assess their needs.  The updating of assessments, care and support plans and submission of funding requests is also a key part of the role. 
  
The roles will be located within the County Wide Reviewing Team and require the ability to travel across the East Riding. 
  
The roles are required for a period of up to 6-months to support a specific project to enhance the review process. 
£up to 35 per hour.
