Pay rate £30 per hour 
Contract role, Hospital Team 
 
We are recruiting for an experienced Adult’s Social Worker in Manchester 
Details: 

• Social worker required for the hospital discharge to assess south team.  

• To support with reviewing care needs for citizens on discharge from hospital.  

• To complete Care Act assessments and Mental Capacity assessments where required.  

• To support all of the pathways out of hospital including reviewing care packages in the community, supporting discharges from intermediate care beds and reviewing citizens discharged into pathway 3 settings such as residential and nursing beds.  

• To also support crisis response to prevent citizens at risk of hospital admission and our hospital at home service.
